- Please include this form with all new degree proposals. It will be best completed in the context of the curriculum development process for the degree(s).
- DEGREES: Board of Trustees (BOT) Cover Sheet
(NOTE: Updated Form must accompany all submissions after October, 2021) - All new degree programs must be approved at the Board of Trustees before advancing to ICHE. The BOT Cover Sheet is the sole document presented. Therefore, the proposing campus should prepare this document and include it with the program proposal at the time of submission within APPEAR.
- DEGREES: Course Articulation Planning for New Baccalaureate Degrees (Appendix 8)
- For all new baccalaureate programs, an articulation plan for students from Indiana's 2-year institutions (Ivy Tech Community College and Vincennes University) is required. Please complete the Appendix 9 document to be included with the proposal.

- CERTIFICATES: IU Certificate Proposal Form
- Use this document for all new Stand-Alone or Area Certificates.
- For new stand-alone certificates to be offered with financial aid eligibility, ALL sections of the IU Certificate Proposal Form must be completed for working with the campus Office of Student Financial Aid to add the certificate to the Program Participation Agreement with the Federal Student Aid division of the US Department of Education.
- ALTERNATIVE CREDENTIALS: Proposal Form
- Use this document for Alternative Credentials (including microcredentials).
- These are approved at the campus and requested for submission into APPEAR in support of recording, reporting and information.
- Program Change Request
- This PDF form is available for items other than new degrees and certificates. When uploading into APPEAR, please also upload any required campus and contingent approval documents, as noted in the Required Approvals Matrix.
- Dual or Accelerated Degree/Certificate Programs
- This Microsoft Word form will expand to the entered content. Please include sufficient detail within each section, though not necessarily that which might accompany a new degree proposal. Curricular detail should highlight coordinating elements across programs and any savings to students. Please note that if either program is offered online, contingent approval from the Office of Online Education is required before the proposal may be submitted for consideration.
